Throughout your search and transition, your Elderwerks Advisor remains available to: • Answering questions • Provide additional referrals • Assist with changing care needs • Review community options • Offering education and advocacy We remain committed to helping you make informed decisions throughout your aging journey. Our Services Are Always Complimentary Elderwerks does not charge individuals or families for our services. Our nonprofit organization is supported through grants, charitable contributions, fundraising efforts, and partnerships with qualified senior living communities and service providers. Because our mission is to serve older adults and their families, we are committed to providing objective guidance that places your needs first. Preparing for Your Move Each senior living community establishes its own admission requirements and health screening procedures. Before moving into a community, you may be asked to provide: • A recent health assessment or physical examination • Medication and medical history • Immunization records or other required health documentation • Tuberculosis (TB) screening, if required • Financial documentation • Physician orders or additional medical records
