Examples may include: • Personal care assistance • Meal preparation • Medication reminders • Transportation to appointments • Grocery shopping • Housekeeping and laundry • Companionship • Care coordination • Other agreed-upon responsibilities To allow flexibility as care needs change, consider including language such as: "The caregiver may also perform other mutually agreed-upon services as the care recipient's needs evolve." Schedule of Care Document: • Days of the week care will be provided • Hours of service • Estimated number of hours per week or month • Any on-call or emergency responsibilities

Compensation Clearly identify:

• Hourly rate or salary • Payment schedule (weekly, biweekly, or monthly) • Reimbursement for approved expenses, if applicable
