MMAI – MEDICARE/MEDICAID PLAN
How to Enroll – Enrollment is voluntary for a 90-day period. At the end of the 90 days, passive enrollment begins; members who have not chosen a plan will automatically assigned one by the State. Choose a health plan, and choose a doctor or clinic to be your Primary Care Provider (PCP) There are 3 ways to enroll: 1. Online – www.enrollhfs.illinois.gov/ node/7 3. By mail: Fill out the form you got in the mail. Then send it back in the enclosed envelope. If you don’t have the form, you will need to call to request one. Most people with an HFS Medical card must enroll in a health plan. Here are some reasons why one may not have to enroll: • Live in a nursing home
Medicare-Medicaid Alignment Initiative (MMAI) is for seniors and persons with disabilities who are enrolled with both Medicare and Medicaid. This program covers all your Medicare and Medicaid benefits, including prescription drug benefits (Medicare Part D). This program is operated by Medicare (the federal government) and Medicaid (the state government). Enrolling with an MMAI plan will ensure you receive better coordination of care and will eliminate the need for you to have to worry about 2 sets of healthcare benefits. If you currently have insurance, and it’s not considered high level, you can enroll in MMAI. If you have high level third party insurance, you do not qualify and can’t enroll.

• Currently in Spend-down Program
• Receiving temporary medical benefits • Receiving care in the Illinois Breast and Cervical Cancer Program • Currently have private insurance that pays for hospital and doctor visits To opt out of MMAI, call Illinois Client Enrollment Services at 877-912-8880; 866-565-8576 (TTY)
