PREPARING FOR HOSPITALIZATIONS & DOCTOR’S APPOINTMENTS UTILIZING A PATIENT ADVOCATE

Doctor’s Appointments

Patient advocates act as a liaison with healthcare professionals ensuring proper treatment for their clients. They can attend doctors’ appointments to ensure their clients are asking questions and are following the Orders by the doctor. These professionals are also important when you enter a hospital. 1. Make sure the hospital and/or your doctor is responsive to your needs. 2. Provide bedside companionship and support. 3. Assist with billing and insurance issues. 4. Communicate with the doctors and nurses. 5. Address any patient safety concerns. 6. Assist with the treatment plan. 7. Assist with discharge options. 8. Coordinate and provide medical oversight. 9. Take charge in a medical crisis or change in health. 10.Provide transportation to appointments. 11. Provide you with peace of mind so that you can get better. They will:

1. When you go to see your doctor make sure to bring:

2. Your medications and dosages.

3. A notepad and pen for taking notes or use the “notes” app on your phone. 4. Discuss why you’re there (write all your aches, pains, concerns, etc. down to help keep you on task). 5. A list of questions to address your current issues. Being prepared will help you with your care plan.
