North West Housing Partnership Programs nwhp.net Aging in Place Program The Older Adult Home Modification Program (OAHMP) is to enable low income elderly persons to remain in their homes through low-cost, low barrier, high impact home modifications. The goal of the program is to reduce older adults’ risk of falling, improve general safety, increase accessibility, improve functional abilities, and increase independence in the home. This will enable older adults to remain in their homes, that is, to “age in place,” rather than move to nursing homes or other assisted care facilities. The Older Adult Home Modification Program integrates services from an occupational therapist (OT), a registered nurse (RN), and a handy worker that work together with the senior to set goals and to increase independence, and safety in the home. Participants learn new skills and exercises to improve function and safety. The services of the occupational therapist, registered nurse and handy worker are free of charge.
